MEDIA RELEASE FORM
Our production will be audio and videotaped and possibly televised.
Parents MUST sign MEDIA RELEASE FORM
and adult (example: parent, aunt, grandmother, etc.)
must bring this form to the performance.
Everyone who attends must consent and SIGN BELOW and agree to be part of the taping and media process and give permission of the unlimited use to allow every child who is unable to attend to see and hear the story and production of ALWAYS BELIEVE.
I agree and accept as part of the 100% volunteer effort there will be no compensation offered for our participation.
Please complete and bring this form to the performance. We will have extra forms at the ticket table for anyone who needs them. We can only provide tickets to those who sign this media release.
